New Patient Registration
If you are a new patient you should bring two forms of ID one with a photo the other with your name and address i.e. a utility bill, with children we need to see their birth certificate. You register with the practice but you can see any GP. You will also be asked to complete a medical questionnaire and be invited to make an appointment for a basic health check with one of our nurses.
You have the right to express a preference to receive services from a particular practitioner or class of practitioner either generally or in relation to any particular condition and we will record this preference. We will endeavour to comply with this but please note that we need not do so if the preferred practitioner has reasonable grounds for refusal to provide services to you or does not routinely perform the service in question within the practice.
YOU MUST COMPLETE THE GMS1 AND RELEVANT QUESTIONNAIRE PACK FOR EACH PATIENT.

As of 29th June 2020 new guidance is that you will automatically be opted in for Organ Donation. If you do not wish to do so please contact www.organdonation.nhs.uk or call 0300 123 23 23.
These can be emailed back to the practice at firstname.lastname@example.org
During this time of Covid we cannot provide certain online facilities. Once we resume normal working practice we will be in contact with you, in order for this to be facilitated.
