Q.O.F.
From 1st April 2004 GP's in the
U.K. have been working under a new contract. This new
contract aims to improve patient care and reward GP's
for the quality rather than the quantity of their work.
About a third of your GP's income
is now based on a "Quality and Outcomes Framework" or
Q.O.F. This framework has 146 performance indicators on
which practices are now measured totalling 1000 quality points.
This is the fourth year of the new
contract. It has been made a lot tougher to achieve
targets with restrictions in government spending but
again we have achieved a very high score of 999 from a
possible 1000.This is due to
our hard work as a practice and of course, you as
patients benefit as a population by being healthier.

Below are some examples of the
indicators, the target required and how the practice
scored between 1st April 2006 and March 31st 2007.
New areas in 2006-07 to be
introduced into the framework include atrial
fibrillation, chronic kidney disease and dementia.

Performance
Indicators
CHD 6. The percentage of patients
with coronary heart disease in whom the last blood
pressure reading (measured in the last 15 months) is
150/90 or less.
- Target 70%. We achieved 91%
CHD 8. The percentage of patients
with coronary heart disease whose last measured total
cholesterol (measured in last 15 months) is 5 mmol/l or
less.
- Target 60%. We achieved 82%
DM 12. The percentage of patients
with diabetes in whom the last blood pressure is 145/85
or less.
- Target 55%. We achieved 69%
BP 4. The percentage of patients
with hypertension in whom there is a record of the blood
pressure in the past 9 months.
- Target 90%. We achieved 95%
BP 5. The percentage of patients
with hypertension in whom the last blood pressure
(measured in the last 9 months) is 150/90 or less.
- Target 70%. We achieved 76%
MH 2. The percentage of patients
with severe long-term mental health problems with a
review recorded in the preceding 15 months.
- Target 90%. We achieved 98%
ASTHMA 6. The percentage of patients with asthma who
have had an asthma review in the last 15 months
- Target 70%. We achieved 73%

Overall Comments
The practice was well
prepared for the QOF Visit. Other data requested on
the day, was also easily retrievable. The team
would like to thank the practice for their
hospitality on the day and also thank the team
members involved for their time and honest
approach. The practice is fully aware of the minor
areas that need working on before year-end to
achieve maximum points. The practice is to be
congratulated.
